Managing Myopia: Frames, Lenses and Contact Lenses

The risks of myopia progression are lifelong and significant, as discussed by optometrists and ophthalmologists in mivision’s special annual myopia issue. Put simply, increasing levels of myopia increase the risk of potentially blinding eye conditions later in life, including retinal detachment, glaucoma, cataracts, and myopic macular degeneration.

While eye care professionals were once confined to prescribing traditional single-vision lenses that only corrected blurry distance vision, now we can potentially slow myopia progression using highly technical, evidence-based spectacle and contact lenses, sometimes in combination with other treatments.

The Evidence for Spectacle Lenses

The spectacle lenses currently approved in Australia by the Therapeutic Goods Administration slow myopia progression by manipulating how light enters the eye.

Clinical studies have shown that these lenses can slow myopia progression by around 50 to 70% in many children, making them one of the most exciting developments in paediatric eye care in recent years.

Orthokeratology: The Overnight Solution

One of the most fascinating approaches to myopia management is orthokeratology (OK).

These custom-designed, rigid, gas-permeable contact lenses are worn while sleeping.

Frames Matter

Naturally it’s easy to get caught up on lens technology when discussing myopia control spectacles. However, equally important is the frame to carry the lenses. To ensure lens efficacy, it must be comfortable, functional, and durable.

Myopia control lenses rely on the treatment zones being carefully positioned. If the frame is poorly fitted or constantly slipping down the child’s nose, these treatment areas may not align correctly with the eyes.

A suitable frame should sit comfortably and securely, provide adequate lens depth, and maintain stable positioning throughout the day. Medium-sized frames with good coverage are ideal. Most need at least 10 mm above the pupil position to work in conjunction with a myopia control lens design.

In short, the frame isn’t just a fashion accessory – it’s part of the treatment. And, because the majority of myopia control will be implemented during a patient’s most active childhood and adolescent years, it needs to survive playground adventures, sports activities, and the occasional accidental drop.

The Importance of Good Dispensing

Myopia control dispensing requires more precision than traditional spectacle dispensing.

Accurate measurements, careful frame selection and attention to fitting details all play a role in achieving the best outcomes. Optical dispensers need to ensure that lenses are correctly centred and that frames remain stable during normal daily activities. Small adjustments can make a significant difference.

One of the most important aspects of myopia management is communication and education. Working together with families will get the best outcomes.

Involving children with clear and simple explanations about how their glasses or contact lenses are helping their eyes stay healthier can improve willingness to wear. Remember that these products work best when worn consistently. Encouraging full-time wear and helping children feel comfortable and confident in their glasses is key to long-term success.

Finally, while advancements in optical treatments have transformed myopia management, lifestyle factors still play an important role. Combining outdoor time, regular screen breaks (the 20-20-20 rule), and advanced optical options gives children the absolute best defence against progressing short-sightedness.
