The use of intraoperative optical coherence tomography (iOCT) has become commonplace across a range of ophthalmic procedures, helping to visualise, in further detail, treatment of eye disorders, including epiretinal membrane, macular oedema, posterior hyaloidal traction, DMEK and retinal detachment. Access to detailed real time OCT images allows users to benefit from informed decision making which can, therefore, enable them to perform procedures with a greater level of confidence.
In a recent case report, Prof Jodhbir Mehta (Singapore National Eye Research Institute) demonstrated how he took advantage of iOCT in the treatment of an iris cyst. In this instance, a 61-year-old male with a past ocular history, presented with gradual onset of blurred vision in 2021. Further examination revealed a large iris stromal cyst.

The patient subsequently underwent iOCT guided iris stromal cyst aspiration and absolute alcohol injection. Prof Mehta outlined how the iOCT system (RESCAN 700, ZEISS) aided the accurate placement of the needle into the cyst and allowed real-time visualisation while the cyst was aspirated, ensuring all contents were removed. Furthermore, the same iOCT features supported controlled inflation with alcohol. This case clearly demonstrated the efficacy of iOCT in reducing the risk of collateral damage to adjacent corneal endothelial cells.
