
Guide Dogs NSW/ACT has launched a new digital platform called Navigate Your Journey (NYJ). The platform aims to support people newly diagnosed with an eye condition causing low vision or moderate vision loss.
The initiative comes after extensive market research revealed significant unmet needs in this patient population.

“Patients have told us they are not always ready to take in all the information provided at the time of consultation and often have many questions arise after diagnosis,” a Guide Dogs statement said. NYJ is designed to complement clinical care by providing tailored information to help patients continue living independently.
The research highlighted that 88% of respondents would use an accessible central platform to find information on low vision and services. Additionally, 94% saw value in having resources tailored to their specific needs, and 68% wanted access to a social/peer support group.
“Finding information is hard. I don’t know where to go or if I can trust it,” one respondent commented, highlighting the importance of a trusted source. Others noted feelings of isolation and the desire to relate to others in similar situations.
Three out of four respondents to the survey said they would find it valuable to have access to psychosocial support during their sight loss journey, with 58% saying they would prefer this service to be face-to-face.
How It Works
Patients can personalise their journey by sharing information about themselves and goals. They immediately receive a ‘My Sight Guide’ with tailored content sent monthly based on their preferences. All data is securely managed.
“The goal is to equip, support, and empower each client to live the life they choose and stay independent for longer,” Guide Dogs said.
Topics covered include understanding eye conditions, using technology, overcoming obstacles, maintaining independence, support services, community resources, funding, transport, and more.

Eye care professionals can also explore the platform and provide feedback to lowvision@cfeh.com.au as it is a pilot project. Flyers are available by email for practices to distribute to patients.
